Form EPW-5 — Manual register & digital reconciliation

  1. Enter the temporary paper reference in the local warning continuity register. Never reuse a number.
  2. File EPW-1 through EPW-4 together with the controlling authorization copy/record location. Maintain the master copy and distribution log as one file.
  3. At network restoration, create or locate the digital warning record. Enter the paper reference as continuity provenance in the administrative notes/evidence packet; reproduce the source reference, public text, times, distribution actions, cancellations and supersession exactly.
  4. Verify the competent source again before any digital state is marked authorized or issued. The digital source gate must still pass; paper issuance does not bypass it.
  5. Record the digital identifier below, the reconciliation operator, date/time, and a digest or scan reference for the paper packet. Preserve the paper original under the applicable records schedule.
